We want to make sure the language we use on this website is clear and easy to understand, and we have created this glossary to explain some of the words and terms which are commonly used to describe our local health and care services.

You can suggest new words or terms to be added to our glossary using the form at the bottom of the page.

  • Pathway

A care pathway is a step-by-step procedure or roadmap which determines how a particular condition or conditions should be treated.
  • Primary care

Primary care is usually the first point of contact for health care. It is mainly provided by GPs (doctors' surgeries), but it also includes community pharmacies, dentists and optometrists (eye health).
  • Primary Care Network

Primary Care Networks (PCNs) are local collaborations of GP practices, organised by the practices themselves to ensure that together they are better equipped to meet the health and wellbeing needs of their populations.
If there are any words or terms on this website which you don't understand and think should be added to the glossary, please let us know using the suggestion form above.
